Position Overview
Title: Direct Primary Care Nurse Practitioner / Physician Assistant
Type: Part-Time (with potential for growth)
Location: Monticello, IN (In-person)
Compensation: Competitive, based on experience
Status: Hourly

What You’ll Do
- Provide whole-person, root-cause primary care in a supportive, patient-centered model
- Develop personalized health plans focused on prevention, nutrition, and lifestyle optimization
- Manage acute and chronic conditions with a functional medicine lens
- Collaborate with an integrative team that values curiosity, learning and innovation
- Participate in community wellness initiatives and conduct virtual visits as needed
What We’re Looking For
- Nurse Practitioner (MSN) or Physician Assistant (PA-C) licensed in Indiana
- Prescriptive authority in Indiana
- At least 1 year of autonomous experience (additional training provided)
- A curious, growth-oriented mindset and passion for root cause medicine
- EMR proficiency and excellent patient communication skills
